WIAA/WSFA Participating School Forensic Tournament Joint Supervision Agreement Agreement form for identifying the supervisor for forensics tournament participants when two or more schools are to be supervised by a coach from a single school. Wrestling Weight Permit (Middle Level) The form for individual wrestlers to have filled out by a physician establishing minimum wrestling weight. Wrestling Weight List for Matches (Middle Level) Weigh-in form for dual matches with actual weight and physician minimum recommendations. Wrestling Weight Certification Summary (Middle Level) Maintain a list of doctor's minimum recommended weights to protect yourself and your athletes. NFHS Fine Arts Sanction Required to hold an interstate fine arts activity. The Executive Board will determine the makeup of the Committee and make the final selection of committee members. Clock Hour Program Application Apply for a program to qualify for WIAA Clock Hours. Completed applications must be received by the WIAA office no less than 14 days prior to the program in order to be approved.
